線維筋痛症に関する国際学会報告(速報)No19

昨年の3月にウィーンで開催された第1回の線維筋痛症の国際学会に参加してきましたが、残念なことに日本人医師は私一人でした。第2回の今年は、Covid-19パンデミックのため延期に加えてバーチャルでの開催となりました。

 

本日紹介する演題は、「一般診療での線維筋痛症のスクリーニング」というタイトルですが、抄録では日常の臨床現場での診断に役立つツール(2016年)、「線維筋痛症の管理のためのEULAR改訂勧告」(2017年)をはじめとする国際基準と現行の管理ガイドラインの知識を正しく使用すれば、大多数のFM患者の転帰を改善すると述べ、そのためにもプライマリ・ケアの場で患者中心の正しい多面的・集学的アプローチが必要であることを述べています。

 

このように、演者のバッティチョット先生は線維筋痛症の診療に前向きで希望の持てる道筋を示しています。

 

ただし、そこには容易でない現実が横たわっています。それはバッティチョット先生ご自身も認識されているものであると推測しますが、日常の臨床現場が多面的・集学的アプローチをとることは不採算を想定した運営方針が確立していない限り容易ではないからです。

 

もし、プライマリ・ケアを担当する開業医が、このようなアプローチを問題なく実践可能であるならば、たしかに医療費を増幅させることなく医療資源を最適に利用できることになります。

 

しかし、その実践のためには担当医たちの更なる自己犠牲を前提とするものになりかねないことは明確に指摘しておかなければなりません。

 

開業医がどれだけの自己犠牲を払っているかという現状が無視されたまま理想論が展開されているのが残念です。

その点、同じイタリアでも昨日のサッコ大学病院のジョルジ先生の御発表の方がイタリアの医療の現実が率直に指摘されていると思います。医療機関の諸々の負担を軽減する方策が乏しい現状のままであれば、イタリアであるか日本であるかを問わずFM医療は望ましい方向には発展しにくいと考えます。

Summary

 

Fibromyalgia (FM), defined as a chronic widespread pain syndrome accompanied by fatigue, sleep disturbance, cognitive dysfunction and several other associated symptoms (e.g. headache, pain/cramps in the abdomen, numbness/tingling, dizziness, depression, constipation, pain in the upper abdomen, nausea, nervousness, chest pain, blurred vision, fever, diarrhea, dry mouth, itching, wheezing, Raynaud’s phenomenon, hives/welts, ringing in ears, vomiting, heartburn, oral ulcers, loss of/change in taste, seizures, dry eyes, shortness of breath, loss of appetite, rash, sun sensitivity, hearing difficulties, easy bruising, hair loss, frequent urination, painful urination, and bladder spasms) or coexisting painful conditions (e.g. psychological distress, irritable bowel syndrome, interstitial cystitis/painful bladder syndrome, chronic pelvic pain, temporomandibular pain disorder, restless leg syndrome, and systemic exertion intolerance disease ) is a quite common disease with a reported prevalence ranging from 2 to 6% in case of patients attending general practitionelized patients and 14 to 20% in rheumatology consultations. Studies in primary care settings confirmed that up to 1 in 20 patients has FM symptoms with a growing trend due to an increasing knowledge of the disease. However, the wild range of symptoms and the gradual evolution of FM, make it difficult to diagnose the disease in primary care settings and often require different specialist referrals and several instrumental tests. For this reason, studies reported a worldwide median delay in diagnosis ranging from 2.6 to 5 years (4) and a median numbers of referrals of 3.7 physicians before receiving a diagnosis. (5) The continuous passage from specialist to specialist leads patients to receive multiple drugs to treat the single different symptoms, delaying the correct multimodal and multidisciplinary approach, and to undergo multiple instrumental tests amplifying healthcare costs.
With the 2016 revision of the 2010/2011 ACR criteria, authors combined the 2010 “physician” based criteria with the 2011 modified “patient” criteria into a unique set of criteria with the aim to obtained useful tool for daily clinical practice diagnosis. Furthermore, the 2017 “EULAR revised recommendations for the management of fibromyalgia” summarized the last decade of scientific evidence in order to help physician in the pharmacological and non-pharmacological management of fibromyalgia patients. The knowledge and the correct use of the international criteria and current management guidelines would allow to create a patient-centric multidisciplinary approach in primary care setting for the majority of FM patients able to improve outcomes and to optimize use of healthcare resources.

 

線維筋痛症(FM)は、疲労、睡眠障害、認知機能障害および他のいくつかの関連する症状(例えば 頭痛、腹部の痛み・痙攣、しびれ・うずき、めまい、抑うつ、便秘、上腹部の痛み、吐き気、神経質、胸痛、視界のかすみ、発熱、下痢、口渇、かゆみ、喘鳴、レイノー現象、じんましん・うずき、耳鳴り、嘔吐、胸やけ、口腔潰瘍、味覚の喪失/変化、発作、ドライアイ、息切れ、食欲不振、発疹、日光過敏症、難聴、容易な打撲、脱毛、頻尿、痛みを伴う排尿、および膀胱痙攣)などを伴う慢性広汎性疼痛症候群と定義される。

 

または共存する疼痛状態(例:心理的苦痛、過敏性腸症候群、間質性膀胱炎/疼痛性膀胱症候群、慢性骨盤痛、側坐骨神経痛障害、レストレスレッグ症候群、およびシステム障害、 精神的苦痛、過敏性腸症候群、間質性膀胱炎・有痛性膀胱症候群、慢性骨盤痛、顎関節痛障害、レストレスレッグス症候群、全身性労作不耐症など)は、一般開業医では2~6%、入院患者では5~8%、リウマチ科受診では14~20%の有病率と報告されており、非常に一般的な疾患である。(1-3)

 

プライマリ・ケアの現場での研究では、20人に1人の患者がFMの症状を持っていることが確認されており、この病気についての知識が増えてきていることから、その傾向はますます強まっている。

 

しかし、症状の幅が広く、FMは徐々に進化しているため、プライマリ・ケアの現場では診断が難しく、異なる専門家への紹介やいくつかの検査を必要とすることがよくある。このため、世界的な研究では、診断の遅れの中央値は2.6~5年(4)であり、診断を受けるまでの紹介医数の中央値は3.7人であることが報告されている。(5)

 

専門医から専門医への継続的な転院は、患者を単一の異なる症状を治療するために複数の薬剤を投与し、正しい多面的・集学的アプローチを遅らせ、複数の機器検査を受けることにつながり、医療費を増幅させている。

 

2016年に改訂された2010/2011年のACR基準では、2010年の「医師」ベースの基準と2011年に改訂された「患者」ベースの基準を組み合わせて、日常の臨床現場での診断に役立つツールを得ることを目的とした独自の基準セットを作成した。

 

さらに、2017年の「線維筋痛症の管理のためのEULAR改訂勧告」では、線維筋痛症患者の薬理学的および非薬理学的管理において医師を助けるために、過去10年間の科学的証拠をまとめた。 (6)

 

国際基準と現行の管理ガイドラインの知識を正しく使用すれば、大多数のFM患者の転帰を改善し、医療資源を最適に利用できるように、プライマリ・ケアの場で患者中心の集学的アプローチを可能にするであろう。
