喫煙されている方へ
当院を受診するにあたり、すぐに禁煙を開始することは難しいかもしれませんが、禁煙に向けて前向きのお気持ちをお持ちでしたら、安心してご受診ください。
なお、当クリニックの患者の皆様の中にも、かつては喫煙者だった方もいらっしゃいます。
「喫煙者には愛を」をモットーに根気強いサポートを続けた結果、喫煙者数ゼロを達成することができました。
線維筋痛症等の特殊専門外来の新患受付終了のおしらせ
COVID感染後や新型コロナワクチン接種後の後遺症が残っている方は、医師の説明による病状の理解が難しいばかりでなく、医学的管理が困難な症状が継続する可能性があります。
しかも、その症状は、線維筋痛症、筋痛性脳脊髄炎、慢性疲労症候群等の諸疾患と類似しています。これらの原因不明の症状を持つ人々は、医療提供者によって誤解される可能性があり、診断を受け、適切なケアや治療を受けるまでに長い時間がかかる可能性があります。さらに、それによって、病状がさらに悪化し複雑化しているケースを多数経験しています。
そのために、患者も医師もCOVID感染後や新型コロナワクチン接種後の後遺症あるいは、線維筋痛症、筋痛性脳脊髄炎、慢性疲労症候群などの病名の先入観を取り除いて、基本的な生活習慣の見直しからじっくりと対応を協議していく方向性が望ましいと考えております。
当クリニックにおいても、臨床評価や日常的な血液検査、胸部X線、および心電図の結果は正常である可能性が高いため、これらを厳密に鑑別して確定診断を下すことは非常に困難な状況です。しかしながら、幸いなことに、たとえ確定診断に至らなくても、有益なケアの方法を見出し、活用するための工夫を凝らすことは可能です。いたずらに焦ることによる悪循環を回避しつつ、希望を失わずに、通常の一般診療の中で一歩一歩前進していきたいと願っております。

Notice of Termination of Acceptance of New Patients for Fibromyalgia Syndrome （FMS）and Other Specialized Outpatient Services：
People with residual symptoms after Coronavirus(COVID19) infections or after innoculation with the new corona vaccine may not only have difficulty understanding their condition as explained by their doctor, but may also continue to have symptoms that are difficult to manage medically.
Moreover, their symptoms are similar to those of various diseases such as Fibromyalgia Syndrome（FMS）, Myalgic Encephalomyelitis （ME）and Chronic Fatigue Syndrome（CFS）. People with these unexplained symptoms may be misunderstood by healthcare providers and may take a long time to be diagnosed and receive appropriate care and treatment. Furthermore, we have experienced many cases where this has further exacerbated and complicated the medical condition.
For this reason, we believe that it is desirable for both patients and doctors to remove preconceptions about the sequelae after COVID 19 infection or new corona vaccination, or disease names such as Fibromyalgia Syndrome(FMS), Myalgic Encephalomyelitis(ME) or Chronic Fatigue Syndrome(CFS), and to discuss how to respond gradually, starting with a review of basic lifestyle habits.
In our clinics, clinical assessment, routine blood tests, chest X-rays and ECG results are likely to be normal, making it very difficult to strictly differentiate between them and make a definitive diagnosis.
Fortunately, even if a definitive diagnosis is not reached, however, it is possible to identify and devise beneficial methods of care to utilise.
We desire to move forward step by step in our regular general practice, without losing hope, while avoiding the vicious circle caused by unnecessary impatience.

新型コロナウイルス後遺症、及びワクチンによる健康被害の診療を開始しました。
新型コロナ感染症・ワクチンによる後遺症に関わる書類発行に関して。
当院では、いかなる理由が生じた場合においても、コロナ感染症に関わる証明書、診断書などの書類の発行はいたしません。
証明書などの書類の必要性が生じた場合、こちらでの診療は終了とさせていただきます。
上記に同意され、同意書の署名捺印された方のみ、診療を開始させていただきます。

Ⅰ. 一般的なご案内
General Information
１）当院は完全予約制となっております。
Suginami international clinic Our store is fully booked.
2）土曜は初診の受付はございません、日曜・祝日は休診です。
Our clinic is closed on Sundays and public holidays. We are not open on Saturdays for initial consultations.
3）会計は現金のみです。クレジットカードや電子マネーなどはご利用いただけません。
We accept cash only. We do not accept credit cards or electronic money.
4）健康保険証をお持ちでない場合、費用は「診療報酬１点につき１５円」となります。
If you do not have a health insurance card, it will cost yout 15 yen per medical fee unit.
5）原則として成人の方のみを受け付けております。ただし、未成年の方であっても、ご家族がすでに当院に通院されている方は受付いたしております。
As a rule, we only accept adult patients, however, we are ready to accept minors who have a family member already attending this clinic.
6）他の病院やクリニックを既に受診されている場合は、紹介状をお持ちください。
If you have already been to another hospital or clinic, please bring referrals (letter of introduction from the former physician).
７）予約の際、紹介状（診療情報提供書）をお持ちの方の優先枠を設けております。
Priority is given to patients with referrals.
Ⅱ 医療制度のルールについて
About the rules of Japanese health care system
１）重複受診の禁止について
<About the prohibition of duplicate consultations>
健康保険では、同一の病気について複数の医療機関で同時に診療を受けることはできません。（重複受診の禁止）
Under the Japanese health insurance system, it is not possible to receive treatment for the same illness at more than one medical institution at the same time. (Prohibition of duplicate consultations)
２）別の医療機関を受診する際には、前にかかった医療機関での診療が終了もしくは中断していること（の証明）が必要です。（前医が病名を転帰＝治療終了していない場合、その後通院していなくても、前医での治療は継続していると見なされます。）
In general, if you wish to see a different doctor, you will need to prove that your previous doctor's treatment has ended or been discontinued. (If the former physician has not finished treating your condition, you will still be considered to be under their care, even if you have not visited them since.
３）別の医療機関に通院されている場合は、当クリニックを受診する前に、必ず通院中の病院の医師に前もってご相談された後、紹介状（診療情報提供書）をいただいてください。

Ⅲ 診断書その他の医療文書の発行について
Issue of medical certificates and other medical documents
診断書が必要な方へ
For those who need a medical certificate
１）自己都合及び自己判断で通院を中断された方、
最終来院日より１か月以上経過した方の診断書は作成できません。
We will not be able to produce a medical certificate for the patients as follows,
If you have stopped coming to this clinic for your own reasons or at your own discretion, or if it has been more than one month since your last visit.
２）就職や進学のための診断書は現在通院中の方のみ発行します。
Medical certificates for employment and higher education will be issued only to those who are currently attending our clinic.
３）当院での診療は治療と社会復帰を目的とするため、障碍者の認定を希望される方のための障害者手帳交付の認定手続き等は行っておりません.
As the purpose of our clinic is to provide treatment and social rehabilitation, we do not issue disability certificates for those who wish to be certified as disabled or handicapped.
