診察室から:高円寺南診療所の国際対応について

高円寺南診療所では、来年の改元を期して、杉並国際クリニックとして国際化対応準備を進行中です。特に、最近、鍼灸治療を希望する外国人が増え、国際鍼灸師としての坂本光昭氏の将来への期待が高まっています。ただし、診察室外の事務手続き、鍼灸治療室では日本語のみの対応しかできていないのが問題点です。

 

そこで、事務方の野口と鍼灸師の坂本の両名は、日常の業務で定型的に使用しているメッセージ表現リストの作成を急がせております。この作業自体が、これまでの高円寺南診療所の医療業務の質を再検討して総括する上で有効な手段となり得ると考えています。そして、この医療会話表現リストを外国語に翻訳する手続きを私、飯嶋正広が担当します。複数の英語ネーティブのチェックを受けて公表することも考えています。

 

最終的には、野口、坂本の両名も、この業務英語マニュアルによって、実践的な英語対応能力を獲得できるようにしたいと考えております。

Foreign Languagesでは英語をはじめ、私自身が聖楽院での歌唱でふだん馴染みのある他の欧州の言語(フランス語、ドイツ語、イタリア語)で案内しています。他に需要の多いスペイン語も準備中です。アジアの言語も重要であると考えていますが、中国、韓国、東南アジア方面の方、中近東の方の多くはすでに日本語で意思疎通が可能であるか、英語が通じる場合が多いです。

 

以下が最近の問い合わせメッセージです。現在までのところ、英語以外の外国語での問い合わせはありません。国籍は、英語圏(英国、カナダ、オーストラリア)、フランス語圏(フランス、ベルギー、アフリカ諸国)、ドイツ、イタリアなどです。英語の達者な中国語系の方や、その他アジア系の方は、直接来院される場合がほとんどです。

I've had chest pain on my right side on and off since October 2017.

 

I've been to the doctor several times but they said everything was fine. I had CT scans, X-ray, and blood tests. They said everything was mostly normal.

 

But yesterday I had a lot of pain (8/10) with slight fever (37.6) and could barely breathe so I went to the ER - they said nothing was wrong... After about 2 or 3 hours my pain lessened and I could breathe better by itself.

I am contacting you because it has been a week that I am feeling very weak with difficulties to breathe and dizziness, headache, pain in my shoulders and elbows and on my chest with difficulties to sleep. I went to a hospital and did blood, lungs and heart check everything was in order.

This is XX(née Y). You used to be my primary care physician when I lived in Suginami. I now live in Chuo with my husband (also named M).

 

I’d like to request an appointment today if possible.

 

Since January, I’ve been suffering from recurring edema (in addition to other ailments) for which I’ve had many tests and medicines. I’ve also experimented with different diet plans (none of which have been effective). One of the doctors gave me Kampo 114, which cleared the edema , but I also gained 3 kilos in two weeks and now have a lump-like feeling in my lower left abdomen. I don’t think that this is related to the kampo, because I’ve had a general pain in my abdomen for a while.

 

Abdominal pain history:

 

At age 24, I was diagnosed with diverticulosis. This has flared up a couple times in the past, causing doctors to check me for ovarian cancer, only to find the issue was an issue with my sigmoid colon. Last week, I went to a gastroenterologist who x-rayed my abdomen, told me there was nothing there and that I should go to the gynecologist. The gynecologist did a Pap smear and a transvaginal ultrasound and said that there were no abnormalities.

 

My abdomen has been sensitive to the touch for a while and I’ve been treated with acupuncture and moxibustion for that and the edema (since March) The consistent pain is new. I tried a liquid diet for three days, in case it was diverticulitis, but the pain did not subside. As there is no blood in my stool, and my blood tests show no sign of infection (6,100 WBC) or inflammation (CRP 0.13) I suspect it’s still an issue with the diverticulosis. Could it be trapped gas?

 

Please let me know if there is a time today that you could consult me. I will bring my medical records so that you don’t have to do any retesting. If there is no time today, please let me know when the next available appointment time would be

My name is XY, and I am a 19 year old male. I have received a vaccination before my flight to Tokyo, but still need to get the follow-up doses here. I received VAQTA 50 against Hepatitis A and IXIARO against Japanese Encephallitis. The follow-up shots are supposed to be given on Sep. 6th, I have received the original ones on 31.07. Would getting those followups be possible, or would you need to start over? After some research I would also leave out the Japanese Encephallitis shot if it was over 10,000 Yen. I am sorry for the short notice!

I just moved to the Koenji area for school. I want to visit your clinic because I have been feeling really awful the last two months. My main concern is the severe joint and muscle pain I experience everyday, especially in the morning. It's very painful in my knees, ankles, neck, and hands. I also have headaches all the time as well as dizziness, red eyes, and fatigue. All this developed recently and I have never felt anything like it before.
